In order for us to help you determine your child's personality profile we would like a teacher or other school official familar with your child to complete the Student Behavior Survey.
You may complete the form in one of two ways.
- The printable version can be printed, completed, and brought with you to your next session.
- The Online version consists of eleven input pages. As you complete each page, your answers will be emailed to your counselor, who will print a copy for review and discussion at your next session.
Regardless of which version you choose, please complete the entire form. Click the version you would like to complete, to begin.
Thanks for your help in helping us to to help you understand your child better. Make sure to tell the person taking the test to include their name and date where indicated for identification purposes.
